The Aria has sign for Bus Tours, but not all do.
This was an incredible trip. We covered a lot of ground in 3 days with some amazing views and hikes. I couldn't have asked for a better experience. Everything went smoothly from start to finish. Safety was never a concern. My guide, Amber, was fantastic. She was very knowledgeable about the places we went, always positive and always willing to lend a hand (such as helping to set up tents). Highly recommend!

The guide was more focused on making us sit and have lunch than he was with hitting trails and parks, and we were often encouraged to hang out at the RV park pool. Also, "camping" means setting up a tent in a parking lot full of RV's, this does not include actual camping and the tents you are given are complete junk. You will spend much more time being driven in a van, or having sit-down meals 3 times a day, than you will actually exploring or hiking. Extremely disappointing for something marketed as DEMANDING. Oh and our guide went through an insane amount of food waste while not recycling, since G claims to have sustainability values which it clearly does not actually adhere to.
the tour itself was amazing. The national parks do not disappoint. The travel accomodations were very cramped and uncomfortable. I am only 5 foot tall and I was cramped in this 14 passenger van. Food opportunities were limited.

The itinerary provided by tour radar was not exactly how the tour was set up. One of our stops said we would be doing a cruise on lake Powell. That was not provided with the tour. Also the hotels that they said we would be staying at did not line up.
Overall, the tour was very fun and we'll put together. The tour guides were all friendly, and seemed to work well as a team. Accomodations were not bad, but there were better options directly adjacent. Food options were pretty much all fast food, which makes sense for the tour concept. However, dinner options could have been a bit better. We really would have appreciated a little more hiking and exploring time, and a couple of the guides did the best to accommodate that. However, 20 minute stops are just not enough time to get "out and experience" the areas.

The trip on it self is awesome. The parks we visit were stunning. However as I booked with Tour radar, it was not mention anywhere the level of the trip. I knew during the trip that this same trip is advertised as a compfy trip with intensity 2/5 on intrepid website. I found my self with 65 Y+ while Im 35 Y and for the hikes it was inconvenient. Hope it will change on the marketplace.
We were fortunate to have Claire Kredens as our driver guide. She is a gifted leader and could cope with ant situation. Such a tour is useful to learn which cities you might like to re-visit because our time in each is very short. Four and five hour drives every other day is exhausting and maybe the number of places visited could be edited downward. On the whole, I enjoyed the trip altho the age range 15-99 is unwise. 15-80 would be much more realistic.

This trip was run like a school trip, we were told that we had to stick to the schedule rigidly even though I asked several times to visit a different museum/shop. Free time was limited and it seemed like the guide had specific ideas of what he wanted to do and see. Our guide also left us on a subway in New York to get home close to midnight and then we had issues getting checked into the hostel. FYI most tour guides make sure their group gets to the hotel safely and without hassle. Waste of time and money.
From a speedboat to the Statue of Liberty to fireworks on 4th July in Washington to hiking to Yosemite Falls, this trip showcased the very best the USA has to offer. I went on the 21 day Full Cyote trip from New York to San Francisco with my brother. With such a packed itinerary of included activities, it offered great value for money. The guides in every region we visited were excellent. A particular shout out to Ryan, who added in extra stops and experiences based on the group's interests. Other highlights included the Grand Canyon, Washington's museums, whale watching in Boston, cycling across the Golden Gate Bridge and sunset in Griffeths Park. I met a great group of people on the trip, and I wouldn't hesitate to book with the Cyote Trip again.

This was an "8-day" tour starting in Toronto and ending in Montreal. As a New Zealand author of novels and short stories, I have travelled widely (about 23 countries). This is my fifth coach tour, the others being through Europe with Trafalgar and Expat Explore. In comparison, the hotels used by Cosmos were superior and all but one were located near shops, supermarkets and attractions. On the second day, the tour kicked off with a hiss and a roar as we travelled to Niagara Falls and for me, the highlight was being on a large boat at the bottom of the falls. Old Quebec City was another highlight together with its interesting history involving the French and English. Our tour leader, Nadine, did an excellent job giving the history and attending to our needs. The coach driver, Dave, was also very friendly and helpful. My wife and I took advantage of two optional excursions: A traditional sugar-shack Dinner, and Quebec countryside dinner and Montmorency Falls. I would recommend the latter. My first longer coach tour and I wasn't sure if it was going to be for me. How wrong was I. It was a fantastic tour, continually being amazing by the beautiful of all the National Parks. 60 Arizona providers and patients put state on notice that they'll sue over Medicaid fraud

A group of Arizona behavioral health providers and clients say they've put the state on notice of an intent to file multiple lawsuits over the recent Medicaid fraud scandal.

Leaders of the effort say they filed 60 notices of claim — precursors to lawsuits — Friday with the Arizona Attorney General's Office.

The notices of claim, which name the state of Arizona as a defendant, collectively add up to $2.3 billion, said group organizer André Miller, a Mesa pastor and second vice president of the NAACP's Arizona Conference.

Among defendants in the notices of claim are Arizona Gov. Katie Hobbs, the Arizona Department of Health Services, and the Arizona Health Care Cost Containment System, which is Arizona's Medicaid program and often known as AHCCCS (pronounced access), said Miller, who announced the claims outside the Arizona Executive Tower at the state Capitol on Friday afternoon alongside about two dozen supporters.

The 60 notices of claim are about "unnecessary damages" and harms that the state needs to account for, Miller said.

"It's a sad day that we're even here," Miller said. "Valid providers have been suspended. ... Something needs to happen. Providers have lost their homes. They've lost their families. People have gotten divorced. Marriages are strained. So many things have happened."

State agencies and the governor's office did not immediately respond to a request for comment Friday afternoon.

The notices of claim were filed on behalf of 38 Indigenous people who are former patients at Arizona behavioral health facilities and 22 behavioral health providers who were affected by the state's crackdown on massive Medicaid fraud, according to Ayo Glanton, an attorney and behavioral health provider who worked on the notices of claim.

Glanton's contract with AHCCCS was unfairly suspended and terminated in the fall, she said.

Valid providers who were suspended were "Black, Brown and immigrant," and "got lumped in the same category" as so-called bad actors, Glanton said.

"We're not saying the bad actors aren't out there but we are saying that we are good actors," she said. "We're still helping folks without receiving any money. ... We're still helping because that's what we do."

Glanton says that in an effort to stop future fraud, AHCCCS has unfairly punished legitimate providers and has not provided necessary coordination of care for patients who were displaced or lost treatment as a result of the crackdowns.

Genasha Cotton, administrator at Pillars of Life outpatient behavioral health clinic in Casa Grande, is one of the providers who filed a notice of claim. Cotton has not been accused of any wrongdoing but says AHCCCS has owed her tens of thousands of dollars since October. Cotton says law-abiding providers are suffering from delayed payments because of overzealous practices and overreach by AHCCCS.

The massive fraud was announced to the public during a multi-agency news conference a little more than a year ago, on May 16, 2023. AHCCCS leaders estimate the fraud has fleeced taxpayers of up to $2.5 billion, and victimized an unknown number of people, many of them Indigenous, with promises of drug and alcohol rehab that they never received.

An AHCCCS medical director recently went public with evidence that the agency knew about possible fraudulent billing and harm to patients as early as 2021 yet did not take any action to stop it until two years later.

AHCCCS leaders have said they had to put more oversight and measures in place to prevent fraud. In the wake of the scandal the state agency suspended nearly 400 providers for credible allegations of fraud, though about 20 have since had their suspensions rescinded. The state also says 79 providers have been terminated from the agency over quality of care or health and safety concerns.

People trying to get sober died of overdoses or alcohol poisoning, according to court documents and information from the Phoenix group Stolen People Stolen Benefits, which was created to help victims of the crisis.

Arizona is already facing at least three wrongful death lawsuits related to the fraud.  Those who died were Indigenous men: one from the Crow Tribe of Montana and two from the Navajo Nation, which has tribal land in Arizona, Utah and New Mexico. They were between 32 and 44 years old.

AHCCCS has launched a public awareness campaign about the fraud, as well as a web page with information and updates. That information is at  https://azahcccs.gov/SoberLivingFraud

AHCCCS leader Carmen Heredia  told reporters last month  that the state agency is a "completely new" organization that has taken numerous steps to prevent future fraud.

"We have really reformed the agency," said Heredia, who assumed the lead position at the agency in January 2023. "We've tackled the bulk of the problem, we really have."

Tips to complete your child's form :

  • Do not sign your child's form until asked to do so by a passport acceptance agent or employee. 
  • You can apply for a passport book , a passport card , or both documents.
  • You may ask for a larger passport book with more visa pages, at no extra cost, by checking the 'large book' box at the top of the DS-11.

2. Get Evidence of U.S. Citizenship (and a photocopy)

Your evidence must be an original or replacement copy. The document must have the official seal or stamp of the office which issued it. You must submit one of the following documents for your child:

  • Issued by the city, county, or state of birth
  • Lists applicant's full name, date of birth, and place of birth
  • Lists the parent(s)' full names
  • Has the date filed with registrar's office (must be within one year of birth)
  • Has the registrar's signature
  • Has the seal or stamp of the city, county, or state which issued it
  • Consular Report of Birth Abroad or Certification of Birth
  • Certificate of Citizenship
  • Please note you must also provide a document, such as a birth certificate, that lists the parent(s) or legal guardian(s) of the child. Full validity means the document is or was valid for 10 years for adults and 5 years for children under 16.

Paper only : You cannot submit digital evidence of U.S. citizenship such as a mobile or electronic birth certificate. You must submit physical evidence of U.S. citizenship and a photocopy of the document. 

Returning your child's document : We will return your child's document in a separate mailing up to 8 weeks after you receive the new passport.

  • Use a single side of the paper

If you do not submit a photocopy, you must submit a second copy of your citizenship evidence. We will keep this copy for our records.

3. Show Your Relationship to Your Child

You must submit a document that lists the parent(s) or legal guardian(s) of the child. Examples include:

  • U.S. birth certificate (also evidence of U.S. citizenship)
  • Foreign birth certificate
  • Adoption decree
  • Divorce or custody decree
  • A court order

Some documents, like a U.S. birth certificate, show  both  U.S. citizenship and parental relationship. These documents must be originals or certified copies (not photocopies).

You and your child may have different last names, as long as the document showing your relationship to your child lists your full name.

If your name is different than the one on the document showing your relationship to your child, submit proof of your legal name change.

4. Get a Photo ID (and a photocopy)

Both parents or guardians must bring a physical, photo ID and a photocopy of it. If your photo ID is from a different state than the state in which you are applying, bring a second photo ID. 

You must show at least  one  of these photo IDs:

  • Valid or expired, undamaged U.S. passport book or passport card 
  • In-state, fully valid driver's license or enhanced driver's license with photo
  • Certificate of Naturalization 
  • Certificate of Citizenship 
  • Government employee ID (city, county, state, or federal)
  • U.S. military or military dependent ID
  • Current (valid) foreign passport
  • Matricula Consular (Mexican Consular ID) used by a parent of a U.S. citizen child applicant
  • U.S. Permanent Resident Card (Green Card) used by a parent of a U.S. citizen child applicant
  • Trusted Traveler IDs (including valid Global Entry, FAST, SENTRI, and NEXUS cards)
  • Enhanced Tribal Cards and Native American tribal photo IDs
  • In-state, fully-valid learner's permit with photo
  • In-state, fully-valid non-driver ID with photo
  • Temporary driver's license with photo

5. Show More Documents (if both parents or guardians cannot apply)

  • Both parents or guardians must approve that we can issue a passport to a child, and go with the child to apply in person.
  • If one or both parents or guardians cannot apply in person with their child, you will need to show more documents.

Important : Submit  Form DS-3053  and other notarized statements within three months of signing them.

6. Provide a Photo

You must provide one photo with your child's application. Go to our  Passport Photo page  for photo requirements and to see examples of photos. 

  • Do not attach or staple your child's photo to the form. The acceptance agent or passport employee will review the photo and staple it to your form.

7. Calculate Fees

When applying using Form DS-11, you will pay two separate fees - an application fee and an execution (acceptance) fee. You will pay the application fee to the U.S. Department of State, and the execution (acceptance) fee to the facility which takes your application.

  • Add $19.53 to your application fee if you want us to ship your completed passport in 1-2 days after we issue it.

  • Returning the passport to you : Pay $19.53 for 1-2 day delivery. This means you will receive your passport   1-2 days after we send it. Include this fee with your check or money order payable to the U.S. Department of State. Do not submit a return envelope to us with postage pre-paid.
