Students are 'doing everything': IRSC moves nursing clinicals online because of COVID-19

indian river state college virtual tour

FORT PIERCE — Nursing students at Indian River State College are interacting with their "patients" a bit differently these days.

Throughout their clinical study — or internship — students are "clicking" and "passing" medications to their patients, also known as avatars, through an online simulation platform, v-STEM.

Some students interact with avatars while others use unfolding case studies where actors play out different scenarios, said Patricia "Patty" Gagliano, IRSC assistant dean of nursing .

“It’s very cool,” Gagliano said.

With schools closed because of the novel coronavirus, IRSC's nursing program was forced to pivot to online instruction, providing virtual simulations in lieu of in-person clinicals.

Despite the switch to online learning, students are experiencing a very similar day to a typical in-person clinical study at a local hospital or medical facility, Gagliano said.

The night before, students receive the following day's assignment. They are expected to begin around 6 a.m., when they look up their patient, work with a faculty member to create a plan of care and spend the day caring for the patient.

At the end of the day, students and faculty members debrief, reviewing a students' decisions, safety precautions and the plan of care for the patient.

Though online learning and some virtual simulation already was in place for nursing students at IRSC, the college increased its online offerings to ensure students remained on track to graduate and meeting the required competencies, or benchmarks, needed to move forward in their studies, Gagliano said.

Typically, students must complete a specified number of hours to meet Florida Board of Nursing requirements, she said. But because of the pandemic, virtual and simulated activity now are accepted, Gagliano said.

Unexpected benefits

For Heather Brigance, an assistant professor in the nursing program, online clinical simulations have strengthened students' ability to think critically and act independently.

While in clinicals, students' hands-on abilities are limited to the rules of the facility, Brigance said. The decision-making, for example, is more observational.

In a virtual simulation, students are "doing everything," she said. "They're the ones deciding to administer the medication or call the doctor. I didn’t anticipate this positive spin, but students are feeling empowered by this," Brigance said.

For nursing student Angela Ramos, nothing compares to interacting with a patient and practicing a set of skills first hand. But for the 36-year-old, who is expected to graduate net spring, "I see positive to both sides of it.

For one, virtual simulations have removed the "fear aspect of being a student," Ramos said.

In a hospital setting, students often worry if they're doing something incorrectly, she said. Virtual learning, though, is helping students recognize things "we might not have in a clinical setting (and) giving us more confidence because we won't potentially hurt the patients."

Virtual simulations also can grant students the opportunity to work with multiple patients and diagnoses a day, she said, which could help broaden students' scope and knowledge.

A "hybrid approach" to nursing education, Ramos said, would be really interesting.

Moving forward, the use of virtual simulations seems likely because of the positive outcomes faculty and students are seeing from it now, Brigance said.

"I actually do think that this time may shift at least somewhat our perspective on how we do clinicals," she said. "I think we will start to use more simulation."

Learning Support Exemplary Practice Winner 2021

2021 Exemplary Practice Learning Support Winner

The Learning Commons at Tallahassee Community College (TCC) strives to provide intentional and targeted academic support for students.  The Boot Camps at TCC were created to intentionally connect with students before the semester begins, provide specific tips for success in select courses, and finally, take the students on a tour of the Learning Commons building to acclimate them to the academic support environment. These sessions were provided for high enrollment gateway courses including College Composition, Argument and Persuasion, College Algebra, Intermediate Algebra, and General Psychology.  Students who attended these sessions were able to ask individualized questions and discover specific information related to expectations and challenges of these courses.  The sessions are essentially designed to provide “chapter 1” or introductory material from the course to ensure students are confident and prepared before walking into the first class.

Hosting workshops throughout the year is usually a challenge for most learning centers in terms of overall student participation.  The Boot Camps at TCC proved quite successful with large numbers of students attending in both the fall and spring semesters of the previous academic year when the sessions were hosted in person.  In Fall 2019, 99 students attended for a total of 114 sessions.  In Spring 2020, 88 students attended for a total of 101 sessions.  However, in Fall 2020, we switched the Boot Camps to being hosted over Zoom and had a total of 685 students attend.  In order to promote the Boot Camps to students before the semester begins, a flyer (attached in the supplemental materials) is created and distributed, information is posted on social media, emails are sent to course sections, and text messages are sent to students enrolled in the courses.

The primary purpose of the Boot Camp is to connect students to the Learning Commons and to aid these students in being successful in the course.  A large portion of students who attended a Boot Camp in the fall also visited the Learning Commons during the semester.  Of the Fall 2020 participants, 86% were successful in ENC1101, 87% in ENC1102, 81% in MAT1033, 74% in MAC1105, and 82% in PSY2012.  The Boot Camps support the mission of the Florida College System and AFC by promoting these types of success rates, being inclusive, and providing an opportunity to close the equity gap.

The Boot Camps are an innovative and welcome addition to academic support at TCC.  This idea is easily adaptable to any college interested in providing a similar type of experience for students.  The key for this program to be a success is to incorporate a multidimensional marketing strategy and provide quality instructional sessions for students.  The TCC students found the Boot Camps to be helpful as indicated by the survey results (attached in the supplemental materials).  We will continue to provide Boot Camps in future semesters and are currently exploring ways to expand this concept to include more classes and partnerships with our faculty.
